This portion of the application allows us to verify that your child qualifies for this grant!
Families must submit an electronic copy of one of the following documents to verify the child's low-income status. The document must include the child's name and eligibility dates. The eligibility or issued date on the submitted document must fall within 1) the last 12 months, or 2) a future expiration date that aligns with the benefit (SNAP, WIC, Medicaid, etc.) period.
Note: All applications submitted without up-to-date documentation will automatically be declined.
We encourage you to re-apply when you can provide current documentation.
All uploaded applications will be reviewed for accuracy and authenticity. Any document found to be altered in any fashion will be rejected and, in certain cases, referred to the proper authorities for prosecution.
